Hi, I'm Grace!For nearly a decade, I've worked as an Occupational Therapist helping older adults recover from falls, injury, illness, and changes in mobility and cognition. Through those experiences, I've witnessed firsthand the profound impact that a person's home environment can have on their safety, independence, and quality of life.Too often, I meet people after a fall or hospitalization had already changed their lives. That experience inspired me to become a Certified Aging-in-Place Specialist (CAPS) and create Age in Place OT with a focus on prevention rather than reaction.Today, my passion is helping older adults and their families make proactive changes that reduce fall risk, support independence, and create safer, more functional homes. My goal is simple: to help people live safely, confidently, and fully in the homes they love for as long as possible.

What to expect

1. Complimentary Discovery Call

Every home and situation is unique. We’ll begin with a brief phone conversation to discuss your concerns, goals, and current challenges. This allows us to determine whether a home safety assessment is the right fit and answer any questions before scheduling your visit.

2. In-Home Safety Assessment

Your 90-minute in-home assessment is designed to identify barriers to safety, independence, and daily function before they become larger problems. The visit typically focuses on the areas of the home where falls and injuries are most common—including the entrance, bathroom, kitchen, and bedroom. Additional areas may be assessed based on your goals and available time.When appropriate, the assessment may also include objective balance and fall risk screening, as well as cognitive screening to better understand functional abilities and assist with future planning for individuals and caregivers.

3. Your Age in Place Roadmap

Following your assessment, you'll receive a personalized written report outlining room-by-room recommendations to improve safety, accessibility, and independence. Recommendations are prioritized based on urgency, your personal goals, and your budget—helping you understand what changes should be addressed right away, what improvements can be planned for the future, and where simple modifications may make the biggest impact.My goal is to provide practical, personalized guidance that gives you and your family confidence in the next steps—whether that means a few simple adjustments or planning for long-term aging in place.
